A nurse is one of the first points of contact for the patient
with the health care team.In the provision of care to clients,it is
essential that the nurse protects the client's identity,promotes
independence and provides culturally sensitive care.
Examples of supporting a client's
identity:-
- Maintain confidentiality regarding client
information.Communicate regarding patient information only with
personnel directly involved in the client's care.
- Provide care only after obtaining permission.It is crucial to
obtain the client's permission before even touching the
client.
- Provide full information regarding care to be provided to the
client; but,the choice of accepting the care provided is the
patient's choice.Do not try to force or persuade a client to accept
treatment.Clarify all doubts and questions that the client may
have.
Examples of supporting client's cultural
needs:-
- Provide for the client to indulge in spiritual affairs as per
his/her beliefs as long as it is not harmful to the client's
health.
- Consider dietary restrictions that certain communities have
when planning the diet. Eg- Kosher diet in Judaism.
- Consider patient's cultural background and provide for it in
the case of terminally ill patients and for end of life care.
Eg-judaism prefers the presence of a spiritual leader(rabbi) when a
person is dying.
Catholics would like to have reconciliation and holy communion
provided to a dying patient.
Examples of promoting client's
independence:-
- It is important to assess the client's ability to function on a
daily basis and promote independence accordingly
Eg: In a post operative client after laparotomy,each passing day
should involve promoting independence in performing usual
activities of daily living (ADLs) as per the client's tolerance.Do
not let pain affect independence.Analgesics should be provided as
per doctor's orders prior to ambulation and activity.
- For patients after a cerebrovascular attack, it is important to
assess their progress with daily physiotherapy and help them to
eat,ambulate or move about with independence gradually as their
condition permits. A nurse has access to a lot of personal and
confidential information regarding their patients. It is important
that the nurse protects the patient's privacy and confidentiality
,considers cultural beliefs and promotes patients' independence
during the provision of care.
