Major complications of the NMS (neuroleptic maligant syndrome) are
1.   Rhabdomyolysis –it is a serious syndrome in which there is muscle injury and release of damaging protein in blood . rhabdomyolsis is the most common complication in NMS . it occur in 30% of the patients
2.   Acute respiratory failure – due to respiratory muscle rigidity there is complication of acute respiratory failure .it is the second most common complication of NMS . mortality is highest due to acute respiratory failure
3.   Acute kidney failure and sepsis
4.   Cardio-respiratory failure
5.   Aspiration pneumonia
6.   Coagulopathies
In old age or in late stage of NMS death is usually due to respiratory failure ,congestive heart failure and sepsis
