He continuously attempts to remove the oxygen cannula from his nose and says it itches, but allows you to gently replace it several times. Vital signs taken 15 minutes ago were BP 130/90, pulse 88, respirations 20. Vital signs now show BP 120/80, pulse 86, respirations 18.
1. Are any of the changes in vital signs a cause for concern? If so, which ones?
2. Should you be assessing for postsurgical bleeding? Why or why not? If so, where would you assess for this bleeding?
3. Should you remove oxygen from the patient? Why or why not?

1-patient having the vitaal signs Bp-120/80 mm of hg
PR- 86 beats per minute
RR-18 breaths per minute.
Here the vital;s are stable now and witin the normal range.If there is any decline in vitals care must be taken.
2- After the surgery surgical site is examined frequently to examine the bleeding.If it is inguinaal hernia assess the gron site for bleeding.Bleeding is a mojor risk after surgical procedures.
3-If the patient vitakl are stable no need of oxygen.If in case of general anasthesia oxygen must be continued.
4-Inform the condition to the physician is very important,after the surgical procedures some patient may develop this type of confusion as the effect of anasthesia.
