What do you find to be the hardest part of the nursing process aka AAPIE (ADPIE) to incorporate into your daily routine as a novice nurse and/or nursing student on the unit? Why do you think you find it difficult? What have you incorporated in the past or currently, if you are in clinicals now, into your nursing routine to help you in making it easier?

I had find the assessment is the first and difficult step in nursing process because sometimes there may be inadequate informations collected from the patient hence they are not provide correct and adequate information . If patient is not able to communicate we can't get proper information. Sometimes attenders ate not able to provide adequate information.

If the assessment went incomplete or wrong following steps are also went wrong. It can not be help in making correct diagnosis and planning. So assessment is very important for nursing process.

Incorporate in the part of assessment to make it easier that we should collect proper documentation from the patient record for evidence that the patient or their attender providing correct and proper information. Reassess the patient before planning.
