The three elements that make a population health program successful are:

  1. decision-making based on information extracted with the help of technology - The use of data analysis and data science in health management is necessary to make better decisions about the clinical related problem.
  2. Well, segregation of health issues - The entire clinical system can be divided into three tiers. The first tiers have a small clinic at the neighborhood level. The doctor will see the necessity of the escalating the problem to the next level. The next level will be at the locality level. The final will be at the town level
  3. Community engagement - Engagement and involvement of the community in the awareness program are necessary for the success of these programs. There will be increased awareness leading to better services.
