Because the patient has bilateral swelling between the legs that means both the ankles are having swelling as per the query.

The reason for this could be the less protein level in blood like in some chronic diseases like liver and kidney disease. Other reason can be congestive heart faliure, more salt retention. If the swelling is confined to the ankles it could be because of arthritis also.

The treatment in case of the underlying disease needs to be addressed first, the liver, heart and kidney issue must be addressed first. If the swelling is inflammatory than anti inflammatory medication and steroids are given to reduce the leg swelling.
