Yes , the above arrangement will cause shape distortion.

Explanation: generally the shape distortion depends on the allignment of the x-ray tube , the body part and the image receptor. The least shape distortion occurs when the part is parallel to the IR and the central ray is perpendicular to both. Here central ray refers to the central ray of the divergent x-ray beam .

So here , we have the same condition that is part and IR are parallel and x- ray tube is perpendicular. But , the problem rises with the off- center of IR . This misallignment issue causes the shape distortion. If the IR is off-center then we can't observe the actual shape of the object. The following picture will make u clear how the off-center of IR causes distortion:
