Narrative charting is an important data recorded by the nurses in clinical setting. It include the nurses observation about patient condition and activities done by the nurse in the clinical area. Narrative chart should be recorded daily.

It is a written statement which include the actual nursing practice. Nurse should monitor the patient activity, include patient's response towards disease, symptoms shown by the patient. Record it properly on the chart in nurses own words. Narrative note should clearly identify the patients problem.

Then explain the nurses activity and care provided by the nurse is recorded as a story with time. Each activity should written in detail. Document the assessment done. We should also record the care provided to the family. Also record the medicines administered with dose and mechanism of delivery.
