Describe a problem that someone may encounter in life. If a person was following rational problem solving, explain how the problem you identified would be evaluated in the five stages.

Common problem that some one encounters in life is Head ache.

Five stages of Nursing process that how this problem could be identified.

Assessment : This could be identified through both the subjective data and objective data.Subjective data is the data which patient explains his symptoms.So collect all the data regarding the patient symptoms such as throbbing pain, nauseated and the level and intensity of pain.Objective data is the data what we assess as a Nurse.The data could be the feeling of the person that should he notified ,the activity of the patient should be observed during pain.

Diagnosis : Diagnosis should be based on the identified problems.It can be for the pain reduction,nausea,throbbing pain etc.

Intervention: The intervention is based upon the problem such as assessing the intensity of pain with pain scales such as wong Baker's faces pain scale.Providing diversional therapeutic techniques such as newspaper,music,television etc.Provide massages that relieves pain,Also provide some nonsteroidal anti inflammatory drugs, some antiemetic.

Rationale: Pain scales helps to assess the level of pains, diversional therapy helps to divert the mind of the patient,massages relieves pain,NSAIDS relieves pain ,and antiemetics reduce nausea and vomitings.

Evaluation: Evaluate the condition of the person after all the above steps.
