Nursing Diagnosis
Altered/Imbalanced nutrition less than body requirement related
to anorexia ,nausea , chemotherapy as evidenced by hesitation to
eat, sores in oral cavity
Goal:
The patient will be treated for the sores in mouth, induced
appetite in a day or two and maintain a good nutritional status in
a week
Intervention
- Assess the nutritional status of the patient to get baseline
information and plan for care
- Provide small and frequent meals to prevent malnutrition and
improve the quantity of food intake
- Assess the food preferences to encourage patient to eat by
willingness
- Avoid foods which can cause bloating, fast foods, excess oils
and provide a bland diet fit easy digestion
- Administer medications as per order to induce appetite
Nursing Diagnosis
Risk for infection related to low immunity level as evidenced by
administration of chemotherapy drugs
Goal
The patient will be free of infection and lead a quality life
before discharge from the healthcare sector after chemotherapy
Nursing interventions
- Follow asepsis in procedure to prevent infection
- Meticulous hand hygiene to prevent cross contamination
- Assess the vitals like temperature which can help to identify
the infection at the earliest and treat accordingly
- Perform laboratory investigations for CBC to rule out any
infection though WBC counts
- Avoid unnecessary procedures which are invasive which can pave
way for infection
- Encourage patient to have good personal hygiene to prevent
infection .
