All progress notes should be read at the start of each shift and should be written with reference to the previous entry and keep in a chronological order.

It must reflect the client’s communication, behaviour accurately and fairly.

When writing progress notes workers must be mindful of how someone reading the notes will perceive the entries regarding the client, and also should be concise, clear and to the point .

Progress notes are legal documents and should be kept confidential.

The client’s goals are achieved or not should be evaluated and to decide what follow up plan is needed.

Providing this document is an important part because the support workers have a clear idea of the client’s current situation, what support the client will require, or what follow-up activities need to during the shift in order to achieved goals made for client.
