Pathophysiology: Hypothyroidism is a common condition where the thyroid gland does not produce enough thyroid hormone which control how the body uses energy and can,therefore,affect every organ system.When the body does not have enough of these hormones,all of the body functions begin to slow down.
Subjective Data: Fatigue,weakenss,sensitivity to cold,irritability, depressions, decreased libido,memory loss,muscle cramps,joint pain/stiffness,constipation
Objective Data
Dry hair and skin,hair loss,weight gain,bradycardia
Nursing Diagnosis
Intervention: Imbalanced nutrition evidenced by weight gain,decreased appetite and sedentary active life
Goal: Client will maintain a stable weight and takes in required nutrients
Intervention:
Defficient knowledge : evidenced by limited questioning about hypothyroidism and taking thyroid hormone replacement.Lack of information about the disease and its management
Goal: Client and family members will correctly verbalize about the disease and replacement of thyroid hormone replacement.
Intervention: Assess the knowledge about hypothyroidism and hormone replacement,Provide information about the disease,Instruct the client to take medication in empty stomach and take in morning to avoid insomnia,teach the expected benefits and side effects.
Fatigue: evidenced by lethargic,compromised concentration,increased rest requirements,unable to complete required activities
Goal: Client will identify the basis of fatigue and area of control ,verbalize the reduction of fatigue and completion of desired activity.
Intervention: Assess the clients ability of ADL,not daily energy pattern,Assess clients energy level,muscle strength and muscle tone,promote an environmental conducive to relive fatigue
Evalute the progress of all set goals.
