Objective data- the patient looks dehydrated, general weakness, extreme thirst.

Nursing documentation- the patient name xyz(here we should mention patient name) came to ED with symtoms of nausia, vomiting, polyuria, weight loss and vital checked and recorded and shows that bp low90/60(example) and skin are dehydrated and needs for fluid replacement. And for furthur evaluation we need to do laboratory test as advised by doctor.

Nursing goals- replacement of fluid volume is needed in this condition

-monitor vitals frequently

- to improve fluid volume and

-provided psychological support

Implementation- given large volume of fluids in order to improve fluid volume and prevent severe dehydration

-administered ondansterol inj. (example) to prevent nausia vomiting

-Given Education the patient about condition and given psychological support.

-evaluation-the patients looks good and patient himself verbalised that he is feeling good
