pressure injury --- It is an injury in a localized region of body due to friction, or pressure which cause necrosis of the tissue in that region.

Stages of pressure injury are ―

Stage 1. ― skin is non blanched with intact area.Their is changes in colour, temperature , consistency of skin like increase in temperature, redness, firm, painful, edematous skin are clinically observable.

Stage 2. ― Loss of thickness of skin partially in dermal area due to which it result in formation of blisters or may be ulceration.

Stage 3 .― Loss of full thickness of skin completely resulting in necrosis or infections of tissue which further extends more deeper.

Stage 4. ― Complete loss of thickness of skin and exposure of bone, muscle which causes sloughing of that area and spreading of infections to other body parts.

Unstageable ― Complete loss of thickness of tissue with sloughing in injured region and deepness of infection get loss.

Suspected deep tissue injury . ― Skin portion which is necrosed got discoloured , it is highly painful due to blisters formation and cause difficulty to observe.
