SITUATION

Hello Dr. Smith. This is Nancy, the nurse on the 9A unit, caring for Mr. John Brown in room 920. I am conforming that he is perfectly alright and has no symptoms till now.

BACKGROUND

He has no symptoms since he has arrived to the uni and has CABG.

ASSESSMENT

I found him lying in the bed totally fine. He states that there is no pain today but has taken regular medicines. His temperature is normal and the incision site looks good, but his blood pressure, heart rate, and respirations are very little elevated.'

RECOMMENDATION

Can we do a regular checkup of his? blood glucose test, BMI measurement? We can also ask him about any change in his health.I’d like to get an EKG and labs, and I need for you to evaluate him right away.
