3. You are at your patient bedside and you are taking vitals. The patient blood pressure is 190/111. What is your next action? The second reading is close to the first reading, what questions would you ask your patient? After gathering subjective data from the patient, what is your next action?

• 1.Orientation- to look over all work and environment .
• Time management - plan everything under fixed time slot .
• Divide the work as per need and emergency
• Utilising skills in proper way like a manager , advocate and caregiver.

2)If patient is denying and there is no emergency then wait for him while performing other works so that patient also get time to sleep .If a nurse would force a patient then patient will get fustrate so to avoid this leave patient for some time . Secondly if a nurse can't skip that patient then explain the rationale to the patient why you need to be do assessment now .

3)First action is to record the bp on notes

If second time bp will same then ask the patient about history of BP.

Second action would be give fowler position the doctor.
