Poor and inaccurate record keeping not only affect the patient care but also lead to legal claim problems as inadequate and inaccurate medical records create problems in the medical claim setting and the reimbursement of the payment through the insurance agencies and thus trigger the whole incidence towards the legal issues. Further it can cancel the license. This leads to improper medical billing and may lead to fraud, which is again the very big legal problem. Hence it is advice to have the accurate, precise and transparent record-keeping with proper nursing assessment with the help of multidisciplinary team avoiding such legal problems along with the delivery of the better and effective care in the healthcare system with utmost transparency and reliability.
