The physical and mental state of each client differ. All the health information of the client has to be considered in the individualized support plan. Each client should be dealt with a specific carer in the long term until the client is healthy enough. Therefore, carer assigned for each client is necessary to have a record in the individualized support plan. Knowing a person better improves support outcomes. This improves the quality of care. Continuous engagement by a single carer allows rethinking in the care plan for the specific client.
