- Date and the time of the procedure
- Explain the procedure to the client
- Charater of soiled dressing drainage color,amount,odour,and
other assessment data
- Assessment findings of wound, size,including length
,width,depth,temperature,pain,approximation of incision or suture
line,surrounding skin condition
- Cleaning of wound and surrounding area
- Application of dressing via sterile technique
- Empsy the drains if applicable ,note the amount and colour
- health education given
- client response to the procedure
