You are a nurse attending to a patient who was admitted into the hospital for severe anemia and fatigue since having a gastrectomy. What conclusion would you make regarding the reason (root cause) for the patient being severely anemic and fatigued? Please type answer.
Complications of post gastrectomy syndrome include Anemia as a result of vitamin B12 or Iron malabsorption and osteoporosis. These problems occur months or even years after gastric surgery. In gastrectomy, proximal stomach is resected, where Intrinsic factor is produced, which absorbs Vit B12. Therefore, in gastrectomy, Vitamin B12 malabsorption occurs due to the absence of Intrinsic factor causing Pernicious anemia.
Normally, acid is necessary to convert dietary Iron to a form that is more readily absorbed in the duodenum which is stored in larger amounts in the bone marrow where red cells are produced. In gastrectomy, Due to the removal of stomach leading to decreased production of acid that causes Iron deficiency anaemia that occurs after a longer time of gastrectomy. Due to anaemia, patient is generally fatigue.
