A Option is not correct because each axis needs a label, a descriptive label is needed to indicate which variable is being represented on the axis.

B option is not correct because each axis need a scale to plot the data. But the low point on scale may be zero or any round number but should be smaller than the smallest data point.

C option is incorrect because it is not compulsory to label the individual data points. It is used to make detail graph and it is very complex.

D Option is correct beacuse it is not compulsory to give title to the graph. Graph may either be titled or captioned. Caption is necessary to provide a brief description about the graph.
