Thiazide diuretics acts by inhibition of sodium and chloride ion transport in DCT.It also causes loss of potassium ions leading to hypokalemia.Hypokalemia produce hyperpolarisation of pancreatic beta cells.This hyperpolarisation will not allow opening of voltage gated calcium ion channels.Because of the absent calcium ion influx,exocytosis of insulin from beta cells is affected.When there is decreased insulin release, there will be hyperglycemia and decreased glucose tolerance which will unmask the latent diabetes mellitus.This can be avoided by supplementing potassium ions along with the diuretic. Thiazides also cause increased plasma levels of triglycerides and LDL cholesterol and cause hyperlipidemia
