1. A patient comes in with fever (42C) and rigors , complaining with CVA pain. She has been bedridden , what would be the best treatment for her?
IV Fluoroquinolone (inpatient) or Ciprofloxacin (outpatient) ? why
* i understand both can treat it pyelo, but why is one better than the other?
2. Patient who is 64 , sputum is bacteria positive CX shows something left lobe of his lungs. Hes startedon Cefuroxime. ALSO a pneumococcal vaccine is ordered for him
Which of the following if the primary reason for the pneumococcal vaccine? and why?
Comorbid COPD or Left lower lung pneumonia?
*My tought process is this, Severe COPD can be seen on xray , and you can treat it with that antibiotic , and you get the vaccine to prevent pneumonia later on
But pneumonia will definetly show on an xray and the antibiotic can also be used. But would u give the vaccine when he still has pneumonia/is being treated for it?**
1)Patient with a very high grade fever of 42C with rigor needs immediate attention and treatment. It is best to admit her and administer IV .Fluroquinolone .This will bring down the fever.
2)Elderly patient has a greater risk of developing pneumonia and other respiratory infection. If the patient has COPD to avoid major problem pneumococcal vaccine are administered.
3)Yes ,the vaccine can be given during antibiotic treatment, rather than I'll effects it provides better outcomes by preventing any additional lung infections.
