yes medical clearance is required. because she may undergo hypoglycemic state. so she need some diagnostic evaluation to confirn the glucose level and need to maintain diabetic sliding scale also to maintain glucose. as ankle swelling is there so she need some some serum electrolyte investigations and need to maintain intake output chart for edema.the risk stratification practice is required to overcome the upcoming risk and to obtain the final diagnosis also. with all she needs counselling also. change in dietary pattern and sometimes patient may required physiotherapy also.
