Write a conversation between a doctor and patient to take an accurate PMH addressing its components as covered and discussed throughout unit 3. You must pay close attention to the following criteria:

  1. The conversation must include eight open questions.
  2. The answers to the questions shall be full and meaningful sentences.
  3. You are NOT allowed to copy from the textbook at all.
  4. Doctor must investigate the FH first, then to address six components of the PMH
  5. Doctor must adopt a patient-centered approach and must show empathy
  6. Medical jargon can be used, but it must be explained to the patient.

The conversation shall be based on the following medical scenario:

You are the doctor. A 41-year-old female patient, who has been suffering from lupus for many years, has been readmitted to hospital. Examination shows that she has cirrhosis, and needs a transplant surgery as fast as possible.

(Medical scenario) Features:

  • FH – father: deceased, stroke
  • FH – mother: deceased, Multiple Sclerosis MS
  • Medical history/ past illnesses: Winter 2010 diagnosed with lupus
  • Childhood illnesses: chickenpox and measles.
  • Immunization: up to date
  • Surgical procedures: she had three miscarriages and two stillborn babies. She had a colostomy in 2008.
  • Allergies: NKDA
  • Medications: OTC – NSAIDs and immunosuppressants - azathioprine
  • Prognosis is very good if she has the liver transplant surgery.
