Write a conversation between a doctor and patient to take an accurate PMH addressing its components as covered and discussed throughout unit 3. You must pay close attention to the following criteria:

1. The conversation must include eight open questions.
2. The answers to the questions shall be full and meaningful sentences.
3. You are NOT allowed to copy from the textbook at all.
4. Doctor must investigate the FH first, then to address six components of the PMH
5. Doctor must adopt a patient-centered approach and must show empathy
6. Medical jargon can be used, but it must be explained to the patient.

The conversation shall be based on the following medical scenario:

You are the doctor. A 41-year-old female patient, who has been suffering from lupus for many years, has been readmitted to hospital. Examination shows that she has cirrhosis, and needs a transplant surgery as fast as possible.

(Medical scenario) Features:

• FH – father: deceased, stroke
• FH – mother: deceased, Multiple Sclerosis MS
• Medical history/ past illnesses: Winter 2010 diagnosed with lupus
• Childhood illnesses: chickenpox and measles.
• Immunization: up to date
• Surgical procedures: she had three miscarriages and two stillborn babies. She had a colostomy in 2008.
• Allergies: NKDA
• Medications: OTC – NSAIDs and immunosuppressants - azathioprine
• Prognosis is very good if she has the liver transplant surgery.

Doctor: Hello FH! how are you feeling now?
FH: Hello doctor, I am feeling frustrated and not feeling well due to my high pain level on the abdomen that is increasing everyday.
doctor: don't worry! I will help better to settle with your problem.
Doctor: FH do you have any other symptoms that disturb you at present?
FH: Yes doctor I am not feeling hungry,i am having nausea and feeling itching in my skin and it is easily bleeding and my legs are swellon for last one week.
doctor: do you lost any weight recently?
FH: yes doctor I lost 10kg over the last 2 weeks
Doctor: FH do you take any over the counter medications along with prescribed medications?
FH: yes doctor, i used to take ibuprofen for my pain.
doctor: did you tell doctor before you are taking this medication?
FH: no
doctor: FH your liver got damaged and you have cirrhosis of liver due to lupus condition that make worst your liver.
Do you know what is cirrhosis?
FH: No! please explain
Doctor: Lupus cause blood clot in the vessels that supply the liver with blood. Have you heard about the transplant surgery?
FH: No doctor. that is the only treatment for this liver disease?
doctro: Yes, because you are so much anemic and look pale and yellow, you need immediate transplant surgery. but nothing to worry...
Doctor: Do you have any insurance plan?
FH: no doctro
Doctor: don't worry, (encourage them) we talk with an insurance company and let you the financial solution for your treatment.
