Rectangle represents Entity and Ellipse represents attribute.Here I took Patient,Doctor,Insurance carrier and Bed Charges as Entities.All the Values in the ellipses represents attributes.Double ellipse represents multi-valued attributes ie;as a person can have two or more contact no.'s. 'Specialist in' is a multi-valued attribute.Diamond represents relationship as there exists a relation between Patient,Charges and Insurance carrier by a common node Billing. Treats represents relationship between Doctor and Patient.Treats is a one to many relation(1:M) ie; A docor can treat many Patients.
