I am looking to create a survey about the condition of roads in the local area...

I am looking to create a survey about the condition of roads in the local area please come up with 12 examples of questions I can put into this survey. The goal of this survey is to determine the conditions of the roads of people in the area. I want to determine what people think should be done about these roads. It should determine major issues the roads have and the type of roads.

If bad road condition, then what is that? a. Pot holes b. Slippery road c. Damaged divider d. Steep slope e. Any other ( Specify)

If any discomfort is there? Yes or No

What is discomfort? a. Congestion b. Unavailability of Sight distance c. Rough pavement d. More holes and cracks in the road e. Any other ( Describe).
