Case management is a collaborative process of assessment ,
planning , facilitation , care cordination , evaluation for options
and services to meet an individual's and family's comprehensive
health needs through communication and available source to promote
patient safety , quality of care and cost effective outcomes.
There are four key components that make successful case
management , which include intake, Needs Assessment, Service
Planning, and Monitoring and Evaluation.
The main four principle of case management
include autonomy, beneficence, nonmaleficence, and
justice.
