1) According to hospital process. Management have to double check the files of all patient to ensure that all the papers and documents are correct and in sequence. But, Carol do not check the file.

2) One of the top management issue is forwarding the wrong information/administration record of the patient. They send the another's patient report to the rehabilitation facility.

3) The acute hospital management do not cross-check the information and failed to notice this discrepancy on admission.
