Mrs. White is taking Entanercept for her Rheumatoid Arthritis. She has been scheduled for a cholecystectomy in 2 weeks. Because of her upcoming surgery, why might it be recommended that she stop the medication ? If the medication is placed on hold when may she resume it?
Entanercept is an immunosuppressant medication that inhibit the production of tumour necrosis factor ( TNF) in patients with rheumatoid arthritis. It acts as anti-TNFs or TNF blockers.
It is better to withhold the immunomodulatory medications prior to surgery because it may present unique challenges for the perioperative period. Also it increases the risks of serious infections like opportunistic infections by bacteria, virus, mycobacterium, invasive fungal, parasitic or other opportunistic pathogens. This may cause hospitalization or even death.
So it is better to stop the entanercept medication one week before surgery.
And it is important to restart the medication postoperatively after 10-14 days. It can also depend on the wound healing.
