From my opinion the covering nurse pick up the provider call and said him that I am a covering nurse whom you are speaking now she told him that agency nurse has gone for a lunch and told her that patient assessement data are in system if provider calls then you open these data and told her what he/she asks, then she open the data and said him to ask question to her  but when provider asks about the condition of patient and how abdominal incision looks today. Now this is a question which can be answered in a phone call for which she make a conference call with a agency nurse and said him to reply provider's questions because due to many abbreviations used in the assessemnet data she is unable to understand it carefully.
