Being a caretaker, the responsibilities never ends with the discharge of the patient. There should be continuity of care. inorder to have a continuity in care, nurse should involve their family members while providing care and also while giving health education so as to involve them in care of patients. This will help the patient after discharge in follow up care. caregiver can increase her competence by educating the family more skilled in participation of patient care even at discharge. This will help in reduction of hospital stay, rehospitalization and also the exacerbations.
