Tina is caring for an infant who is running a very high fever and vomiting whenever Tina tries to feed him some water to keep him hydrated. If she continues to give him water to try to lower his body temperature and increase hydration and, as a result, he vomits more fluid than he is drinking, then he will only get more dehydrated, and his body temperature will rise. What do you think Tina should do to reduce the child’s fever, make him more comfortable, and enhance his ability to drink liquids?
Tina should check the temperature of the infant if a thermometer is available and record it.If the temperature is too high she should consult a paediatrician as there is a chance for febrile seizures if temperature goes beyond 100.4degree farenheit. So if you are sure about the usual dose of antipyretics like Syrup Paracetamol 120mg/5ml give it after consulting with the paediatrician.By the time other measures can be take to reduce the temperature which includes remove the clothing of the child.Wear very loose cotton cloth and give the baby teppid sponging with Luke warm water for 20-30mts.
Temperature can be because of infection as well as dehydration. If the baby tolerates breast feed the baby frequently and if it's not tolerating Oral Rehydration Therapy should be started.Rehydration solution sachet is available in grocery stores also well also medical shops and it doesn't need any medical prescription for the same.Rehydration solution replenishes the lost minerals as well as restore fluids.Rehydrationsolution should be prepared as per the instructions in it and can be used for 24hs once it's prepared.Rehydration solution should be given every 15mts for the first 2hrs and also breast feed in between as tolerated.Monitor the temperature after 30 mts and if the condition continues the same make an emergency consultation with the Doctor.
Watch for warning signs of dehydration and febrile seizures which include eye lids swollen or sunken eyes, less frequency of urination, dry mouth etc.
