Personal health record is defined as the collection of records of the patient's personal health and all the information regarding the patient is maintained in the personal health records.

Increasingly patients are creating and maintaining personal health records with data from the variety of health care providers as well.as data they have generated about their health.

Understanding the terms, privacy and security is very important when it comes to provision regarding a patient's policy related to the privacy and security of the PHR 's.

Privacy is the right of the individual to control access to their person or information about themselves. Security relates to the administrative , technical and physical safe guards implemented to prevent privacy and confidentiality breaches.

Certain laws have to be in place to ensure a person's privacy and security. An organisation should notify individuals when it comes to collection, use and maintenance of their personal health records

The purpose of security and privacy of personal health record is to keep the privacy of confidential information about the patient.

The provision or information that should be included in the personal health records are

Name of the doctor and phone number

Allergies such as drug allergy

List of diseases and surgeries with the respective dates

Medications including dose, route of administration and frequency of administration

Chronic health problems such as Diabetes mellitus, Hyper tension, Hyperlipidemia etc.

Immunization history

Family history

Living will or advance directives.
