Family history , chief complain with duration,surgical history, medical history,personal history, vaccination,allergy or smoking ,alcohol use etc.

then physical examination TPR and assessment of respiratory system by inspection(Inspect prsence of lesion,infection,rashes), palpation (palpate the prsence of abnormal mass, symmetry, tumor ,lump scar and swelling),percussion (to know the high pitch and low pitch sound.Ex a hollow sound suggest bronchitis.),auscultation (to know abnormal breathing sound, plural friction in pneumonia).
