One of your site supervisors makes contact to inform you that a subcontractor suffered serious injuries when a section of rebar fell, during a crane lift, and struck him on his shoulder and back. Describe the immediate actions you would take and outline the main elements of the subsequent incident investigation you would lead, in order to establish the root cause of the incident and the main contributing factors.

The Immediate actions that we should take are-

• Arrange proper medical attention for the injured person.
• Provide fund for the required medical bills on behalf of the subcontractor.
• Arrange a paid leave for him, as he was injured on working condition.
• Appoint another temporary subcontractor to look over his unfinished job, so that the work can go on.

Main elements of the subsequent incident investigation-

• The lifting crane condition should be investigated immediately.
• The crane operator is to be questioned.
• The authority in the charge of crane maintenance should be called on.
• The site supervisor is to be questioned about the site safety, required changes in safety system should be done.
• A meeting may be arranged with the workers whether they are satisfied with the safety arrangement of site or not.
