The variable is categorical because different categories such as 'no use', 'ocassionally' etc are the values that the variable can have. The values can be ordered, making it ordinal level of measurement.

2. Obesity status - Categorical (dichotomus) variable and Nominal level of measurement.

The variable can have categorical values, not any numbers. Therefore, it is a categorical variable. There is no increasing or decreasing order. Therefore, it is nominal level of measurement.
