In a multi-tier health care system the minimum coverage individuals at the bottom of the tier system should receive is a prudent amount allotting for better service to society, either enabling the people to work or serve, as Buchanan noted on page 746. This basis of coverage should allow for people to maintain health and preventably treat possible ailments. This type of obtainable system should improve overall wellness and allow for individual's unable to afford excessive care the ability to continue, or prolong their capable life of contributing to society. Hopefully, this would transpire a decrease in unemployment and a boost in economy. Providing a minimal amount of coverage to members of society unable to pay would promote Buchanan's theory of enforced beneficence, compelling tax payers to be responsible for the minimal portion of health care to be offered via the government.
Daniel's view on the tier system focused on the doctor's responsibility to patients. He talks about the different obligations doctors are obliged to meet, but also the risks they assume through treatment. If taxes that are collected by the Government can be taxed on the basis of annual income, then the rich will be paying tax that is affordable amd the poor will be paying relatively lesser.
This tax can then be used as goodwill money, by government, to help open a fund for people with financial troubles, and there could be a system in hospitals that gives waiver as per 'need' and amount a person can pay.
So Healthcare would be provided to people who cannot afford it, but the people who can will be charged normally.
