2). Lack of appropriate infrastructure and electronic information.

3). Determining which measures or outcomes to be taken

4). Lack of standard methodology for adjustment of risk related to comorbidities and complexity.

5). Lack of appropriate plants to measure the outcome as a part of routine care.
