Description of the client - the client seeking our service is a homeless man (in his 40s), seeking some assistance with housing. He has a history of substance abuse disorders, and some underlying physical conditions.

The possible services this client might need are - emergency shelters, drop in centers, housing that is permanent, psychological help, counselling, food stamps etc.

Assuming I am the case manager, I would try to coordinate all the needed services even though they may not be provided by my own agency. E.g.. my agency does not not have in-house psychologists and so counselling service will have to be outsourced or managed with the help of a different agency.
