i think various department of health care are isolated as there is lack of communication or sharing of information between two sites of health care.This fragmented heath care make the cost of health care high and also make it difficult to fully resolve problem related to health patients receives care through a patch work of provider of varieos site- primary care practices,specialty clinics and others which doesnot communicates.

Now certain steps has been take for make cordination within health care but it is still lacking coordination
