Changes sought in the aftermath of the Columbia disaster were not sustained. As it can be seen that NASA was still dependent upon federal funds which were an external factor beyond NASA’s control. The main component which needs to be changed is the culture which remained the same as it is still not concerned about safety. Management was still not giving importance to the problem of lack of communication and feedback from minority groups. Organization rules were still not followed and decisions were taken informally. The organization still did not focus on key areas for change instead it wanted faster cheaper and better program.
