Wells Fargo problems arose due to "really bad" incentives it had given to the bank. the incentives were for meeting account opening targets. The incentives problems were worsened by lack of feedback. employees then went ahead and opened accounts in customers names without their permission leading to $190 million settlement with U.S. authorities
