The launch phase of the organization development is a very crucial phase, wherein the success or failure of the development project lies in this phase. Following are some of the important pitfalls or mistakes that must be avoided during the launch phase of the Organization development.

1. Disconnect or discontinue communication. There must be continuous flow of information and all stakeholders must be well informed during this phase.
2. Assumptions must be at minimum. This would ensure one sees the potential threats to the project clearly.
3. Becoming lethargic or slow reactiveness - The launch stage requires the team to be responsive even to the slightest hint of threat.
