Toyota is the best example of that organization who choose the wrong project managemnt structure that lead to problems. As Toyota board of directors consist of 29 japanese members , all of them are Toyota insider. As a reasult of its centralized power structure, authority is not generally delegated within the company , all US executives are assigned a japanese boss to mentor them. and no Toyota executives in the united states is authorized to issue a recall. most information flow to one way , back to japan where decisions are made. Hierarchy of seniority are at a risk of reacting to external threats slowly .
