While discharging a combination of two communication models should be utilized-

  1. Verbal instructions- Verbal instructions on diet, general precautions like walking, bathing, sleeping etc. should be explained to the patient if he is in a condition to listen, understand and ask queries to clarify his or her doubts.

  2. Written Instructions- Written instructions about how to take medicines, when to come back to the hospital for any subacute care or examination of the physician etc.
