Preoccupation of failure is the first high reliable organization (HRO) doctrine. As per this doctrine, company needs to pay continuous attention to the anomalies, which can be indicators of larger problems in the system. There are 3 ways of how HROs deal with the failures:

• HROs strive hard to detect small, emerging anomalies, as they can be indicators of other anomalies and failures in the system.
• HROs analyze and document the mistakes so that the same may not be repeated in future.
