NITROPRUSSIDE

It may interact with nitrate receptor in the smooth muscle cell membrane. Increases cGMP level which may relax smooth muscle by help in forcing out the calcium out of the muscle cell. This result in the dilation of the arteries and veins, which reduced peripheral vascular resistance.

The nitroprusside is metabolized into thiocyanate.

Adverse effect

1. Anxiety, dizziness, headache, restlessness

2. Hypotension

3. Nausea, vomiting, diaphoresis, twitching

4. Flushing and rash.

Note- in the ICU unit the healthcare provide should see the thiocyanate toxicity as the nitroprusside is metabolized into the thiocyanate.
