The physician orders 250 mg of an injectable medication. The medication vial reads 500mg/ml. You prepare the injection and administer it to the patient. As you complete the injection you realize that you injected 1 ml of the medication. What is the problem with this injection? How do you manage the situation?

Here the doctor advised to give 250mg injection.

Dise in hand is 500 mg /ml

Hence the quantity to be given is

Orderd dose /dose in hand ×quantity

250mg/500mg ×1 ml = 0.5 ml to to administer.

But instead of 0.5 ml nurse administerd 1 ml of injection.

It is a medication administration error.

  1. Medication errors must be informed to the physician as soon as possible.
  2. Carefully observe the patient for any side effects and complications.
  3. Explain it to patient and reassure.
  4. Maintain a patent intravenous line ,and IV fluid.
  5. It should be report as an incident .
