How does the health belief in Germany compared to the UK?

#. Health Care in the UK based on :-

- NHS -National Health Service (NHS)

- BEVERIDGE MODEL

- PER PT

- PUBLIC funding and tax based so that everyone pays tax and gets part of NHS ( so dont really get a bill is paid in the taxes ).

- public control over most hospitals

- regulation of docs

Doctors in the UK:

GPs are part of PRIMARY CARE TRUSTS

- in UK HAVE A LOT OF PRIMARY CARE

- have LOT of control... 1) capitated, 2) can send pt to specialist and 3) can still get bonus

Primary Care trusts

- set budgets and services

- contract with specialists

- paid by funds from government

- pay for performance (movie and british doctor).

Have major reform in way to have " commission groups" to allow docs even more control.

Describe the health care model in the UK

-LOT SIMPLER

everyone pays general taxes that go to NHS then have 1) expanded capitation to doctors with negotiated contracts to specialists and hospitals and 2) global budget to hospitals via negoitated contracts.

- hospitals now have to compete for the contracts and also for pt. if lose pts then the hospital is not funded.

Private sector in the UK:

About 12 percent of the population has private health insurance

- they pay for health insurance in ADDITION to paying taxes to NHS so point is in UK CANT opt out of paying public tax to NHS NEED to pay that now in addition can buy extra

- offers choice of specialists, less waiting time for elective surgery and non urgent care, and higher standards of comfort in private facilities.

specialist physicians can practice in BOTH public and private care delivery sectors

ex: if have pt that did surgery was private but if chronic prob after now public.

* SOCIAL INSURANCE (BISMARCK MODEL )

- instead of everyone paying into tax put aside money yourself and when need it can use it.

- germans are insured through MANDATORY not for profit regulated SICKNESS FUNDS everyone must have a sickness fund.

- funds are quasi- public

- employers AND employees required to pay into a sickness fund in EQUAL AMOUNTS

- funds are inclusive, no experience rating, rates are regulated, and MUST offer all medically necessary care, plus Rx, dental care and preventive care. *broad and comphrenseive.
